Presbyopia, or age-related farsightedness, is a process that accompanies the natural aging of the body and develops gradually after 35-40 years of age. Presbyopia is expressed as the inability to focus at distances up to 30-50 cm. It has peculiarities in people with refractive anomalies:

  • Patients with myopia see better in the distance;
  • Patients with hypermetropia have a significantly worse vision.

Treatment is needed in both cases since presbyopia does not correct refractive anomalies, but rather can lead to complications up to severe retinal and corneal diseases. Today, age-related farsightedness treatment remains possible and feasible.

Treatment methods for age-related farsightedness

Treatment methods for age-related farsightedness are quite different. Some may reduce the symptoms, while others relieve the pathology for years to come.

Presbyopia: medical treatment

If presbyopia is getting worse, the medications do not help for long. These treatments cannot stop the process, but only relieve symptoms and slightly correct your vision. Medications for age-related farsightedness include:

  • If presbyopia advances, these drops help soften the crystalline lens. It is believed that its loss of elasticity is the main cause of presbyopia, so these drops slightly slow down the process. Today, other progressive options are being actively researched. It is possible that droplet treatment to restore the elasticity of the lens will soon be more widely used in presbyopia, but for now, there are more effective correction methods.
  • Supplements and vitamin complexes. This is a group of drugs that contain several valuable substances that are essential for good vision. These are mainly vitamins A, B, E, as well as zeaxanthin and lutein. Supplements contain herbal ingredients that support the visual organs. Basically, such products have no evidence base, and their effect on presbyopia is not reliably known.

Glasses selection

In the case of presbyopia, there are several options for glasses, being the traditional method of correcting the pathology. This is the cheapest method, which is designed for specific tasks:

  • Reading glasses that allow the patient to see well at close distances are chosen;
  • Additionally, distance glasses are needed to see clearly at the far distances;
  • Bifocal glasses with two optical zones, combining both options for close and far distances.

Initially, a person may experience psychological discomfort while getting used to the glasses.

Multifocal lenses

Also known as progressive contact lenses. They cost more than glasses and require regular replacement, but many people prefer this option because of its benefits:

  • Multifocal lenses are more aesthetically pleasing;
  • Patients note that lenses are more comfortable compared to glasses;
  • Multifocal lenses have a smooth transition between the optical zones, so one sees well at any distance, and does not experience discomfort when switching between “far” and “near” focus.

Modern lenses are available in any variation of the replacement mode (day, month), have high oxygen permeability and moisture content for comfortable wear.

Surgical treatment for age-related farsightedness

Surgical correction for presbyopia is limited to lens extraction and multifocal intraocular lens (IOL) implantation. Modern trifocal lenses allow seeing well at all distances, both far and near. The artificial lens is made of a special polymer with high biocompatibility with human tissues. Therefore, after implantation and extraction, the patient recovers quickly.

Laser correction of age-related farsightedness

Laser correction of age-related farsightedness is a technique widely used in Europe and is available in Ukraine at Eximer Ophthalmology Clinic. The correction is performed using:

  • The German SCHWIND Amaris system with PresbyMAX® program for the Presby Lasik technique;
  • IntraLase FS60 laser for the unique medical technology of Presby FemtoLASIK.

This method appeared back in 2009, and today, it keeps getting better. This unique technology has several advantages over traditional presbyopia treatment:

  • It corrects old age sight to the maximum comfortable stage;
  • It simultaneously corrects any form of refractive error such as myopia, hyperopia, and astigmatism;
  • It corrects two visual pathologies at once and is carried out in a single day;
  • Comfortable vision at all distances; increased acuity without loss of contrast;
  • It has been used worldwide for over 11 years and has high ophthalmological ratings. Patients never come back with complaints of vision deterioration even several years after correction.

This technique is based on a biaspherical multifocal ablation profile, which allows maintaining a high quality of vision. The ablation zones have an aspherical profile that optimizes corneal curvature for all visual areas, including close, medium, and far vision. A high-precision technological system estimates each eye individually, what zones, and how deeply the cornea should be corrected to ensure good vision at any distance.

In each case, the surgeon chooses the optimal PresbyMAX technique, for example, in some situations, a technique that focuses on improving close vision is known as PresbyMAX µ-Monovision.

This surgery is performed using the brand-new IntraLase FS60 laser, the most advanced equipment in laser ophthalmic surgery. The corneal flap is formed completely by the laser beam in a few seconds, without additional mechanical exposure to the microkeratome. Therefore, the laser allows performing surgery even on the thin cornea and reduces the risk of postoperative complications.

Studies show that presbyopia after laser correction ceases to be a problem in the vast majority of cases. In the early postoperative period, 78% of patients achieve satisfactory vision at different distances. And six months after the surgery, 97% have high visual acuity at close and far distances, which considerably improves the patients’ quality of life.

Age-related farsightedness: surgery and contraindications

When age-related farsightedness arises, you should be aware of the contraindications before making a decision about surgery. Concerning the presbyopia correction, here are some of them:

  • Pupil dislocation (1.0 mm from the center and above);
  • Reduced pupil response to different light conditions;
  • Cataract, which can only be fixed by lens extraction;
  • Glaucoma, which is known as high intraocular pressure;
  • Retinal dystrophy;
  • Corneal pathologies and diseases.

People with systemic disorders, such as rheumatoid diseases, should also avoid the surgery. It may put you at a higher risk of complications.

If you simply suffer from refractive anomalies and presbyopia, the surgery may be performed.

Presbyopia laser correction: getting ready for surgery

If you already know you have presbyopia and are planning PresbyMax laser correction, start preparing in advance. There are no special conditions, but if you wear lenses, refuse them. Take off your hard lenses 1 month before the examination, and take off your soft lenses 4 days before. This way the doctor can assess the original state of your eyes with high precision, which is crucial in selecting the optimal parameters for the correction.

If the doctor approves laser correction after the examination and finds no contraindications, the next stage is choosing multifocal contact lenses. The patient will need 3-4 days to get an idea of what his vision will be like after the surgery. On the day of surgery, avoid driving a car.

Presbyopia after myopia laser correction

Presbyopia after myopia laser correction is an unpleasant situation, but it still can occur. In fact, a patient with myopia had laser correction at a young age (before 35-40 years old) and by the age of 50, he/she develops age-related farsightedness. This affliction mostly affects people with high myopia. There is still an option of repeated correction. At a young age, the patient gets an opportunity to see perfectly well in the distance, and with strongly progressing presbyopia, additional correction can be performed. But it usually happens 15-25 years after the first surgery.
